Opt-In Site Examples?

I'm building a site offering a free guide to whoever opts-in to my list. I'm pretty new to this so I want to learn from successful examples. If you have an opt-in page or know of any good ones please post their URLs.

I have seen that big arrow pointing to the rectangle with the email forms does a great job. However, I would definitely be more creative. Use ajax code for sliding effects or some cartoon character bear pointing to the newsletter
