To set up your own dedicated these days is really silly. There are so many companies out there that have great dedicated or vhost plans at really decent prices. They handle all the support, the pipeline etc ..... its really the only way to go!

The best way would be to rent or lease a server.
There are too many things involved in setup a server: router, security, etc...

It's not worth the time and trouble.
You can rent a dedicated server ranging from $100+ depending on what you need (manage vs. non-manage, 2nd disk for backup, etc...)

    Yes.

    You would need to setup:
    - Internet connection - DSL, T1 or T3 depending on what you need
    - Router
    - DNS server (may be)
    - Firewall

This is not always the case - many companies set requirements for servers which eliminate the possibility of leasing a server or vhost. There are thousands of companies dealing with regulatory burdens such as HIPPA, Privacy Act and financial services watchdogs that eliminate the possibility of dropping a web site and or application in a third-party data center.

This would include companies outside the US who have US-based customers in addition to the regulatory demands on companies outside the US in financial, medical or real estate industries among others.
