    Question Submission Forms or Forms

    Does anyone know where I can get a free submission form or form template? I want to be able to add in certain information (Your Name, Email Address, City, State, Zip Code, Payee Name, Print Name, and more) that is required for my clients to fill out. Basically, I want to send a created form through email that my clients can fill out and then have them email it back to me.

    I have a styled form example here

    However, that will not process the form for you. You will need a formmail script to process the form. Check with your host- most supply their clients with a free FormMail script.

    It would only be a matter of changing a couple of lines in the script and the styled form, for the form to work. The hosts' FAQ page generally explains these things - check it out.
