echo web form processor not working?

Code below doesn't seem to work. I filled in all the necessary info - used to use the echoweb form so I know how it works, but I'm getting a "Forbidden, access denied" message when I test the forms. Any help?

Are there any other free form processors like this one? Response-o-matic isn't good, I hate all the ads on the thank you page. Looking for something customizable, little or no ads, ability to use own thank-you page.
