    Hi,

I assume your interested in the layout of the form and not the technichalities of the form.

I usually try to keep the form simple and as neat as possible. It's often difficult to layout a form as the text is of varying lengths and then the input boxes are all misaligned.

So I usually turn it around like this:

I put the input boxes first and the text follows. I think this approach is simple and looks neat and is easy to follow. You can colour each alternate row to make things easier to scan.

In the end it all depends on the layout it goes into and what you want it to look like

Use the fieldset and legend to group things together and don't put the reset button right next to the submit button. Leave at least a whole line gap so the user doesn't click the wrong one by mistake.

I'm sure other people will do this differently so the best thing to do is experiment and see what works for you

    Paul

    Edit:

You should of course also add accessibility features such as tabindex to make the form tab through in the correct order.
