Uploading Graphics

How would create a form that uploads graphics into a directory...

i have a table with a field called ID
i need to add all the data inputted by the form
then upload the graphics that have been browsed to on the users computer.

i.e i have a form:

Name
Description
Logo
Shoplogo

i need to add Name and Description into the databaee (i have a auto_increent field called ID)

THEN i need to upload the graphics to /uploads/id

if yuou get what i mea
