Form trouble
I have a form here that works:

Now if I add a select box(very bottom of page) then the form fails. Can someone tell me what is causing the form to fail? It just does nothing after pressing submit. If I delete the select box it works again?
I just started having problems on this host, all my forms that used to work now return this error:
> There are missing fields!
> recipient is missing

> Form Dump:
The recipients are there and the forms work on other servers but when I changed the method from post to get, they worked again?

Well, it just submitted for me?
By the way, I see several form fields with the same names (IE: MAXLENGTH="15" NAME="LiabilityPayment"). This could very likely cause issues...

It does work for me in netscape but not in IE 6?
