Well, at the moment I am helping someone - out of the goodness of my heart - retrofit a whole website to W3C Level Triple-A Conformance and yes, it does fall down own to manual human judgment to some degree.
It is good to simulate situations like using the keybord only and using a greyscale browser, I am doing the latter right now with images turned off.

Absolutely. I don't have any physical or visual impairments yet I find myself using the keyboard more and more to navigate sites and find it very annoying when they don't play ball! Try the Lynx viewer as well, I always think it's important to see what the text only users are seeing.
