Need Help with form

This one should be an easy one, but I have no idea how to do and I have been going crazy trying to figure it out....

All I need is a form processor that will also be able to send an auto response to the person filling out the form.

When the auto response is sent out --- it includes the visitors name - personalized in the email,

Ex:

Dear BOB,

Thank you for filling how the happy kitten go lucky club!!

PLEASE PLEASE help me with this, I have a client that said he would destroy me if I didn't do it.

how can you not know about the mail function of php, and yet be having clients who are paying you to program there web applications?

I doubt many people started getting paying clients when they just started building websites...

And the mail function is quite a well known function, but ah well
